What it is

FOXO4-DRI is an experimental “senolytic” peptide — a molecule designed to selectively eliminate senescent cells. Senescent cells are aged cells that have stopped dividing but do not die; instead they accumulate with age and release inflammatory signals thought to contribute to ageing and age-related disease. The “DRI” refers to a D-retro-inverso design that makes the peptide more stable.1

It rose to prominence after a single, widely publicised mouse study. The most important thing to understand is that its reputation rests on preclinical research only.

How it works

Many senescent cells survive by relying on interactions between the proteins FOXO4 and p53. FOXO4-DRI is designed to disrupt that interaction, freeing p53 to trigger programmed cell death (apoptosis) preferentially in senescent cells while sparing healthy ones.2 The hope is that selectively clearing these cells could reduce age-related dysfunction.

This is an elegant mechanism in principle, but it targets a core cell-survival pathway, and its selectivity and safety in a living human body are unproven.

What the evidence shows

Laboratory and animal studies

The key evidence is a 2017 mouse study (and supporting cell work) reporting that FOXO4-DRI could selectively kill senescent cells and improve several markers of ageing in mice — including measures of fitness, fur density and kidney function.2 This result drove substantial interest in senolytics generally.

Human studies

There are no human studies of FOXO4-DRI. No human dosing, efficacy or safety has been established. The honest summary is a single striking mouse result and nothing in people — a very long way from a validated anti-ageing therapy.
