What it is
Setmelanotide is a melanocortin-4 receptor (MC4R) agonist — a peptide medicine that acts on a specific receptor in the brain’s appetite-regulating pathway. Unlike most peptides covered on this site, it is an FDA-approved, prescription drug, sold as Imcivree.1
Its approval is unusually narrow: it is indicated for obesity caused by particular rare genetic conditions, not for general weight loss.
How it works
Appetite and energy balance are governed in part by the leptin–melanocortin pathway in the hypothalamus. When upstream genes in this pathway (such as POMC, PCSK1 or LEPR) are defective, or in conditions like Bardet-Biedl syndrome, the brain’s “fullness” signaling is impaired, driving severe early-onset obesity and intense hunger. Setmelanotide bypasses some of that upstream loss by directly activating the downstream MC4 receptor, which can reduce hunger and body weight in affected individuals.1,2

Animal studies
Preclinical work on melanocortin signaling established MC4R as a central regulator of appetite and a plausible drug target for genetic obesity, supporting progression to human trials.2
Human studies
Setmelanotide was approved on the basis of clinical trials in defined genetic populations — people with POMC, PCSK1 or LEPR deficiency, and later Bardet-Biedl syndrome — where it produced meaningful weight reduction and decreases in hunger in a substantial fraction of participants.3 Importantly, the evidence supports use within these specific groups, not the general population.

Safety & side effects
A distinctive feature of setmelanotide is its effect on pigmentation: because melanocortin receptors also act on pigment cells, skin darkening (hyperpigmentation) and darkening of moles can occur. Other reported effects include injection-site reactions, nausea and effects on sexual function.1 It is a prescription medicine with specialist oversight and is not appropriate for everyone.
